Description:

3.1. Оплата за цим Договором здійснюється Замовником наступним чином: 3.1.1.Замовник за заявкою Виконавця здійснює попередню оплату в розмірі 30% (тридцять відсотків) від орієнтовної місячної вартості Послуг за кожний календарний місяць, - що становить не більше ________________ грн. (________________ ___________________________________ гривень ____ коп.) з урахуванням ПДВ (для неплатників ПДВ – сума передоплати зазначається без урахування ПДВ). Попередня оплата за перший місяць дії Договору розраховується пропорційно кількості днів надання Виконавцем Послуг в першому місяці. Попередня оплата здійснюється на підставі виставленого Виконавцем рахунку та на строк не більше одного місяця відповідно до належним чином оформленого рішення Міністерства охорони здоров’я України. 3.1.2. Остаточний розрахунок за місяць, в якому надавалась Послуга, здійснюється Замовником на підставі погодженої Замовником звітної документації Виконавця за фактом наданих Послуг та підписаного Акту приймання-передачі наданих послуг за місяць протягом 5 (п’яти) банківських днів з моменту підписання Сторонами такого Акту з урахуванням здійсненої попередньої оплати .
